OFFICIAL RULES
1. NO PURCHASE NECESSARY. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING.
2. HOW TO ENTER: To enter, either (a) visit offbroadwayshoes.com, and complete the official entry Off Broadway Shoes Free Shoes for a Year Sweepstakes entry form, or (b) visit any participating Off Broadway Shoes Store location and complete and submit the official 2011 Free Shoes for a Year Sweepstakes entry form. The sweepstakes begins on November 15, 2011 and ends at 10pm Eastern Time on December 26, 2011. Sponsor shall not be liable for lost, misdirected, garbled, incomplete or illegible entries, or for electronic or computer malfunctions or transmission errors through the internet or mutilation occurring while in the mail. Limit one (1) entry per person. Entries become the exclusive property of the Sponsor and will not be returned. If you provide your e-mail address with your entry and you were born before November 15, 1993, you will receive email notifications of special sales events, coupons and other promotional materials. 3. WINNER SELECTION: One (1) winner will be selected in a random drawing from all eligible entries received. Drawing will be conducted on or about January 6, 2012 in Alpharetta, GA, by Off Broadway Shoes whose decisions are final and binding on all matters relating to this sweepstakes. Winner will be notified by overnight mail, phone or e-mail and will be required to complete and return an Affidavit of Eligibility and Compliance with these Official Rules and a Liability/Publicity Release. Each must be signed and received by Sponsor within 7 days of date printed on notification or an alternate winner will be selected in a random drawing of all remaining eligible entries. If any prize notification letter or any prize is returned as undeliverable or if the Affidavit of Eligibility and Compliance is not returned within the 7-day period described above, the prize may be awarded to an alternate winner in a random drawing from all remaining eligible entries at Sponsor's discretion.
4. PRIZE AND ODDS: Prize is a coupon book with twelve coupons, one for each month for twelve months. Each of the twelve coupons is good for a single pair of shoes valued at up to $50.00 (Fifty Dollars). The coupons will only be valid for use during the month identified on the coupon and cannot be combined with additional coupons of any kind. The coupons may not be used for the purchase of gift-cards and may not be redeemed or exchanged for cash. If the price of the shoes for which the coupon is used is less than $50.00, the difference will not be refunded. If the price of the shoes for which the coupon is used exceeds $50.00 the coupon holder is responsible for the difference. The retail value of the prize is up to six hundred dollars ($600.00). Odds of winning depend on the total number of eligible entries received. Prize is non-transferable. Sponsor reserves right to substitute a prize of equal or greater value. Taxes and fees, if any, are the sole responsibility of winner.
5. ELIGIBILITY: Sweepstakes open to individual legal residents of any state of The United States or the District of Columbia who are 18 years old or older, but excluding employees (and their immediate families and members of their household) of Off Broadway Shoes, their respective affiliates, agents, judges and advertising and promotion agencies. Winner agrees to allow Off Broadway Shoes to use their name, city and state, likeness, and/or prize information for promotional purposes in any medium without additional compensation to the extent permitted by law.
6. GENERAL: This offer is void where prohibited or restricted by law and is subject to applicable federal, state and local laws and regulations. Sponsor is not responsible for lost, expired or damaged Prizes. By participating in this Sweepstakes, each entrant agrees 1) to be bound by these Official Rules, 2) that the decisions of the Sponsor are final and binding and 3) warrants that s/he is eligible to participate in this Sweepstakes. By accepting prize, winner agrees to release Sponsor, their respective directors, employees, officers, and agents, from any and all liability, loss or damages arising from or in connection with the use or misuse of the prize or participation in the Sweepstakes.
7. WINNERS LIST: For the winner's name, send a self-addressed, stamped envelope postmarked by February 28, 2012 to: Off Broadway Shoes, Free Shoes for a Year Sweepstakes, Attn: Marketing, 4300 Alexander Drive, Suite 100, Alpharetta, Georgia, 30022. Requests for the winner's name will be fulfilled after the winner's name has been verified.
